American idol and medical clinical exam

American idol season 10 premiere is back 2 days ago. This is the much awaiting moment for me and a lot of people. Everyone awaiting to see an american idol without Simon which is the most nasty jury among 3. things turn out the new jury- J Lo and Steven Tyler turn out much nicer than Simon. I hope second episode will be more promising as the season goes on. A big NO still a bit reluctant from J Lo.

What is the relation of American idol with medical clinicL EXAM. Actually it's the same, to pass clinical,you need to prepare to perform. just like in Idol, you need 'Proper dress', proper knowledge as your vocal in idol and the most important thing is able to perform. If you get nervous, you need to prepare to resit in the exam. It's you bring the best to jury to get yourself qualify. Although sometime you may meet people like Simon in your exam, you will struggle for it . If you are good luck, then you meet Paula or J Lo then it's easy for you to go though. Whatever it takes, you need to fulfill minimum criteria to pass.

Finally, good luck to participant in Idols and people sit for clinical exam!

I keep my promise to do altenate day call which happens to be non working days. The outcome is taxing. i felt super tired after the call. I won't give up for sleeping at home the whole day. I went to mid valley which happen to be Raya kedua. the people in mid valley is flooded. looks like KL got a lot of citizen now rather than people just stationed for working, or studying.
Resident evil 4is the movie i watched in 3d mode. fighting top rated however still not ended. story wise not really good. I like it because no need brain to processed for this movie. Anyway , it is ideal for post call movie.
I will be recharged and went for the next on call. the third raya call is super bad as a lot of dehydrated with sepsis patient came in. Anyway i think it's rather a bad call than good.

What i can said? I feel deeply sorry as brazil lose to 2-1 by losing the patience after a surprising 2-1. A own goal and lose of patience of melo . A red card all ended the Brazil to qualify for semi final.
A heartache to every brazil Fan including me. A lose means no more Samba football to watch in semifinals.

a nejm article in June 23, 2010 issue a article with the above title. it make a alert to me and many medical profesional. Are we doing good to pt or do more harm to patient by ordering CT scan.
According to this article, Radiation doses from CT scans are 100 to 500 times those from conventional radiography. Technical advances such as increased imaging speed have led to new CT scanning techniques that have also boosted doses. For example, the brain-perfusion scan uses sophisticated techniques for assessing regional blood flow and, even when done correctly, delivers a dose 10 times that of a routine brain CTTill now, it has not been shown directly that CT increases cancer risk, radiation is a known carcinogen, and extensive epidemiologic and biologic evidence links ionizing-radiation exposure with cancer. The National Research Council (NRC) has concluded that patients exposed to radiation in the range provided by a single CT scan have an increased cancer risk.
risk of single Ct is as high as 1/80.
Back to question, are we doing good by doing Ct scan. it depends but i totally disagree of detail CT scan or full body scan for check up if there is no symptom to suggest the source. Detail perfusion scan should be avoided unless indicated.

world cup fever and Exit of main team

from june 11 till now, world cup already enter the knockout stage. Surprising exit for Italy and france( champ and runner up) in last world cup due to wrong formation and playing defensive football.
A major clash of england and germany which i expect a goal difference game end up england was destroyed by germany in the most embarassing way. A 4-1 result prove that england formation is totally wrong. A old defender like John terry is totally no match for germany young player counter attack. A 4-4-2 formation made them died even faster. They need to put in4-3-3 formation with defending midfield to counter germany formation.
Anyway, bye bye to england. Now only left Holland ,slovakia and portugal to played for europe apart from germany.

The solution is G-doc. Upload your pdf file in g-doc either in word, pdf and etc then read in from g -doc. Isn't it great.

A tale of 3 airway=pro and cons

yesterday, I completed my experience in 3 flight company in Malaysia finally. The firefly is my last choice at last in my yesterday domestic travel back to KL. My previous priority always Malaysia airline and AIrasia respectively.
First of all , firefly using a small aircraft- a high-wing, twin turboprop aircraft which make the time spent to get down a plane on arrival reduced as small plane means less pessanger vs big plane. Time to take big luggage out from airplane also reduced. In conclusion time saving.The offering of snack in air flight also make it more competitive compare to its main rival -airasia. Finally is the airport is at the Subang airport which is much nearer to LCCT and KLIA. The cons-i think it is mainly a small airplane,if there is turbulance, shakiness a bit higher than both of its competitor, the steadiness of flight less, the last is the noisiness of the plane .
Airasia which claims to be cheapest air fare among all the air craft is not so cheap after all compare to previously except during promotion period or very early booking time not even 2 weeks prior to travelling. The benefit is it got a very comfortable big plane , very nice magazine to read-the 3sixty magazine which is quite rich in contents compare to setia in firefly sponsored by SP setia (not very enjoying to read) .Another good point is that is a bus in LCCT airport. The cons is that is no snack in flight ,you need to buy it on board and very far airport LCCT.
Mas wise is the flight which provide highest airfare with even Business class in domestic flight-definitely the most luxury flight. Big plane with very quiet engines compatible with airasia. The fare relatively higher of all times however the presence of KLIA express make the travelling from KL sentral to KLIA take only around 30 minutes with a reasonable cost. KLIA is also comfortable to travel with. The Mas magazine is also very nice to read with very good photo and contents. The cons mainly is the price, location of airport and the long waiting time to get down from airplane as the plane is big(similar to airasia) however that is direct corridor connecting plane to airport so no need to use umbrella on raint day like airasia and firefly.
In conclusion that is pro and cons in respective air company. The only deciding factor is your choice and favour of the benefit than bares with the cons.
If you search for cost and comfort- go for airasia
If you search for time and comfort-may consider mas
If lokking for distance of airport, time of going down from flight, cost-maybe firefly.
To me, mas still remain my choice if my wallet not too tight at time of travelling.

3R for our earth rather than football?

today when at the flight, I learn some new term in firefly magazine called 3R-reduce, reuse and recycle.
It is different from what i know about 3R . As i am a football fan, the 3R i understand is Ronaldo, Robinho and Ronaldinho of Brazil in last world cup. Unexpectedly, green concept or i rather called it green trading using protecting earth as a packaging.
Among the 3R in protecting earth, i think reduce is the most important.Reduce is just like preventive medication. By reduce the purchase of whatever not really needed, we can reduce the wastage and rubbish. So why we need recycle if that't not much rubbish.
Reuse to be a bit like treating medication what i do almost daily. After we use the plastic bag than we should not throw it and we can reuse it again and again until it is broken. I actually do it daily. It is a bit like treating the disease when it existed .The concept of this is a bit lower than reduce as rubbish or unfriendly item to earth already existed. The last R -recycle is more like palliative medication for example degradable plastic, etc. Some of the product is not really friendly but just to ease the guilt of the user. The recycle product take more cost, lower quality and maybe more energy consumption than create the new one. The only good point is reduce the unfriedly product in the earth. Green house effect and global warming is not much help.
AS a earth residents, i think we should reduce wastage.

a permanent learning process

I remember once my MO told me during my HO(house officer ) time, in a process of learning procedure , see one, do one and teach one. Even when i am a MO in GH , my consultant also tell me the same time but when i worked in teaching Hospital, my HO don't show initiative to learn procedure ( about 90% of HO population), they more interested go back early then do procedure. It is very sad for current Ho population and worrying . I wondered how do they cope with the procedure when they are MO.
Ask specialist to do? I am not sure.
3days ago, A great teacher of medicine, Prof Izham came to our ward to give his teaching. His enthusiasm towards medical knowledge, advancesu in medicine other than his speciaty really impressing. He said even when you subspecialist in medicine, you should well aware of other internal medicine development, after all . YOU are the physician at base . A physician should always learn the new thing.
The moral of the story, medical field is a permanent learning process even for great professor .Then why our HO show no interest to learn?

Coffee and me

I try to stop my daily coffee 2 weeks ago. It ends up i got the withdrawal syndrome. Every day headache started 5.15 pm and above not relieve by sleeping. I thought it is post call syndrome due to lack of sleep initially but 2nd day, 3rd day post call still got same effect. I decided to drink back my coffee but the oral regurgitation and palpitation back in progress. I in biggest dilemma to drink or not to drink.
At last , i decided back to my coffee habit as I could not bear migraine.
Now i had no more palpitation and all side effect.
Maybe next time i should wear the above T shirt to stop coffee!

PC fair , a total Jam

What is the main cause of migraine? people said it is due to Trigeminovascular system ,Cortical spreading depression , serotonin role and etc.
I find congestion in PC fair by human and human crowd plus loud music in 1 hall selling speaker is one of the causative factor.
Using my low resolution handphone i manage to show how congestion is it.
Especially the HTC corner because they give you the free gift like umbrella (not the rihanna sound track) i mean real umbrella and window T shirt. The price is about 100-150 cheaper i think. THE next biggest attraction i think that is a lot of pretty girl promote their theme in costume. A lot of people busy shooting picture with them.
To me, that is definitely good price for hard disk, GPS, pen drive and printer. Computer wise normally you get free gift more compare normal shop purchase plus you can compare all branded computer on your budget.
Another interesting thing is Pen Drive offer laser engraving on your pen drive. something u won't get on your normal purchase unless you want to use knife to engrave your pen drive.
How about next pc fair, i would go even it cause migraine to me but maybe went on Friday rather than saturday. maybe take leave next time.

HIN1 vaccination -my personal experience

As i am the medical doctor , my risk of getting H1N1 is higher than the public. We are the targeted population for vaccination prior to public.
Hot topic regarding H1N1 is whether it is safe. FDA approved so what? documented side effect Guillain Barre syndrome.Myalgia, arthralgia, flu like symptom and etc.

Balancing my benefit vs risk , I decided to go for it! It is because i highly potential to expose to it and i like crowded place like mid valley. Although i may have antibody to it previously seems like got H1N1 but not eligible for throat swab.

I get the injection yesterday noon.To be convenience and challenging the effect. I decided to take 2 in one sitting-H1N1 and influenza vaccine 09/10. 4 hour later, i got sudden runny nose and warmth on my body. I do my immediate non pharmacology therapy-Taking bath and drink a lot of water. Still continue my daily activity as usual.
today wake up at 8.45 am, strong as usual, manage to go shopping and do whathever thing without myalgia.

LOng term side effect- Guillain Barre syndrome unable to comment but claims to be same like normal population and no increased risk.

Contraindication- allergy to eggs

P/S somebody got vaccination and doing on call without side effect.

To me -go for it . U won't want to be the statistic of death of H1N1

holiday mood and surprise

every year 2 to 3 days prior to chinese new year. It is a major kiosk to ward . All old chinese patient who is slightly better but not completely well will request for discharge even though just admit for 1 days!
This year, similar things also happens, lady with congestive heart failure came in with acute pulmonary oedema just weaning off from BIPAP also request for discharge. We successfully persuade to stay for extra 2-3 days before discharge, today she markedly better due to our promise to let her go back if she is improved or due to antibiotic ? i am unsure?
anyway she recovered well and we fulfill our promise. let her Go back. Just like what Nathyi in avatar told jake sully " go back"

what is the surprise ? Due to severe stress during on call , my colleague admit to CRW secondary to unstable angina. As everyone know, working in health care esp medical line, we have more stress than anyone . Stress during resuuscitation. stress from higher authority for fail to perform or showmanship? stress when old or frail patinet (> 75) died suddenly when antibiotic already given for so called reversible condition. Even more stress when crisis management(failed to explain) to family not yet established.! A top priority stress will be complaint from colleague from other department and worst still back stabbing from some "idiot colleague"
Anyway, if the world is so peaceful, it already named heaven not world.
Bare with it. Hopefully , new Tiger year, it will roar away all the bad luck and people.

I accidentally opened a you tube containing Susan boyle on thursday. I hear a angelic voice singing i dreamed a dream. She look so common just like a normal middle aged lady. When she opened her voice, what a angellic voice come out,i hear it again and again, she got a sweet voice which i liked. Compared her voice with another vocalist Sarah brightman, sarah brightman voice is more sharp and her voice is more powerful

Hurray , i got more vocalist to hear now.

Moral of the lesson: don't judge a book by cover.

Being a dr for quite long, i actually already try to be fair with every pt and rational on my treatment. I actually do not give special treatment to so called VIP pt but rather give priority to more ill pt.
MEdical do teach us judge pt by cover- wasting pt-chronic ill pt, jaundice- liver problem, autoimmune hemolytic anemia if pale. needle mark-never ignore HIV and etc

what is the famous thing for low yat plaza. every one sure think about PC, laptop, handphone and electronic gadget. Recently i find the Tan Tatt( egg tart) sold in Tatt king at ground floor very delicious and nice.
First reason, it is crispy. I never taste a egg tart with so crispy external.
second the inside part also nice.
Price wise 1.80 per tatt. a bit expensive compare to normal price.

My first album was released in 1987. A promo tour was planned that turned into a 15 month tour! I got really lucky. The first single was a hit, then the second.... I was dating my wife now, Cynthia Rhodes, and had been for about two and a half years. She'd already done a couple of things, Flashdance and Staying Alive, and she got a job in South Africa. I'd been on tour for so long that we really wanted some time together. Much to the horror of my agent and manager, I took two weeks off to fly down to South Africa in the middle of her shooting schedule. She'd already been gone for five or six weeks and we really missed being together. This was before email and text messaging and with the time change, it was hard to communicate. And it would be three months since I'd seen her.

It was 1988 and there were (rightfully) sanctions against apartheid and the political situation there. Had I been asked, I would have gladly given my anti-apartheid position, but no one asked me. I got a call three days before I was going to leave that my visa had been denied. I got shut down.

So now I had two weeks off with nothing to do except be miserable and miss her. I called a friend of mine and set up a day to write a song. For a songwriter, songwriting is therapy, only cheaper. I wrote a raging rock and roll song, my version of an AC/DC song. We were just about done, and my friend's phone rang and he excused himself to take the call.

There was a baby grand in another room in the house and I went straight to the piano. I sat down and "Right Here Waiting" spilled out effortlessly. It wrote itself in 12 minutes, the only time that ever happened to me. I grabbed an envelope I found and wrote down the lyrics. My friend heard me singing it and ran and got a tape recorder. He said I had to put it on tape right then. My only mission was to send it to Cynthia in South Africa to tell her how much I missed her. When she got it, the 4 minute song took her a full day to get through. She said that after every line she stopped and cried.

A few weeks later I got a call from Barbra Streisand. She'd heard my music and asked me to write a song for her. After I went home and started thinking about it, I thought about "Right Here Waiting" and decided to give it to her. She loved the music but wanted me to re-write the lyrics. She said, "I don't want to be right here waiting for anybody!"

I told her that I didn't want to, it was too personal. She said, "Why don't you record it?" I didn't want to -- it was too personal for me. I was in the studio recording my second album and everyone who heard it kept on telling me to record it. Very reluctantly, I did. Everyone who heard it said that I had to put it on my album.

The first single was "Satisfied," a rock and roll up tempo song that hit #1. Radio and my fans had jumped on "Right Here Waiting" immediately. That's when I realized that it is a universal song. It went to #1 and stayed there for three weeks. It was phenomenal for me. I framed the chart and sent it to Barbra with a note thanking her for turning down the song. We remind each other when we speak and laugh about it.

After all these years it's still the song in my show that pulls everyone in the room together. The audience is from 12-75 years old and they all sing every word. I start it and then just play the piano and let the audience sing. I'm thrilled to have a song that ubiquitous. Soldiers and their wives have told me, "It's our song." What a tremendous privilege that is!

I wrote it because I missed my wife really badly. It's not our song anymore -- in a good way. It's everyone's song.

I wrote the lyrics to this song when I was 18 and homeless. I had been living in a place I was renting and had a job at a computer warehouse where I answered phones. I had no money at all and was living paycheck to paycheck and still didn't have money for food. My boss propositioned me and when I wouldn't agree, he withheld my paycheck and wouldn't even acknowledge me when I went to his office and asked for it. My landlord was nice, but said that if he didn't get paid he would have to kick me out, which he did soon after.

I thought I would sleep in my car until I could get another job. I got really sick with kidney problems and got infections because I couldn't afford medications or doctors. When I tried to go to the emergency room, I almost died in my car in the parking lot because they turned me away since I had no insurance. One nice doctor saw me and helped me and even got me the medicine I needed.
I'm not proud of this, but I was so broke that I shoplifted. The only thing I took was food -- carrots and peanut butter. I couldn't keep a job because I kept getting sick. Then the car I was living in got stolen.

I was walking by a store window and saw a dress I really wanted. I remember it was $39. I'd never taken anything like that before and when I even considered it, I realized that I must have lost all faith in myself if I didn't think I would be able to afford $39! I knew then that I had to regain self-confidence. That's when I wrote the lyrics to this song.

Life is a calcification of your thoughts. I was watching what my hands were doing. Are they opening or closing doors? Are they shoplifting or writing songs?

My husband and I went camping in the mountains in northern California and as we were coming back down, we noticed an American flag at half mast. We thought a fireman may have perished because fires are not uncommon there. As we came further down, there were more flags at half mast. Finally, the radio worked and we learned that the twin towers had come down. It was surreal. Then we heard the DJ dedicate "Hands," a song I'd written at 18 at a dark time in my life, to America. It was an unbelievable experience.

You have to keep fighting for what you believe in.

today got intention to spent money on new toy, wonder need to buy ps 3, wii or the most urgently need RMVB player. The player which let you interface what you download and play it in your TV. Something cost you 170-200 previously. Now is between 119-150 low end, HDMI -179-220. HD resolution 320. I buy 129 one, good feature, eligent look and most importanly cheap and reasonable for me. No money to buy hdtv , guess is enough for me for this time . Later but hdtv one when buy hdtv, Price may be 120 later. Can get 2 then.
Experience in computing is buy whatever neccesary first coz price go down super fast, tech is on going improve.
Next toy to buy would be vaio-p. wait till i desperate, price is still high 2999-3999.

I actually temp to write this blog 1 week ago but my soul and emotion remain slight depressed plus busy with my work so this blog title is remain running in my mind.Born at 1958 and died at 2009. His age of 50 is a bit young but compare to musical legend, John lennon, elvis. He live the longest. Compare to my patient . He live shortest. For your information, my youngest patient under my care is 67. Average age is 75-85.I think average age of Malaysian will be increase in next cencus.
I occasionally compare my pt with MJ, I think MJ living in glory although late in his life , that's controversy in his private life but his moon walking dancing, unique sound will remain in every one head regardless you like his music or not.
MJ is the first singer who sang english song i know of. I know thriller, billy jean, beat it at primary school. He sentimental song like you are not alone, ben , heal the world are also my favourite.
To me a glory 50 years is much more valuable than living 90 years in miserable and no people knowing about you.
The most important thing in life is living meaningful rather than living long .
I think people nowaday are "dying too long, living too short"

Autobot transform and roll out

I just came back for my long awaiting movies-Transformer-the revenge of fallen. I am a transformer fan since primary school. I like autobot . I identify good and bad by identify the eyes of robot. AUtobot-blue eyes. Decepticons-red eyes. The movie made it easier to identify . Almost all the decepticons(BAd side) is stainless steel colour. How about good side- more colourful.
The stories is quite modified than what i used to watch when i small. Other than the old character and name in autobot. Got some even can change in to human form=? terminator hybrid. Any way overall movie is good. I think third installment will be coming also looking at it's ending.
The next good movie coming will be GI Joe and harry porter.

P/s: not transformer fan maybe a bit diff to differentiate good and bad but may still able to follow it. Fighting scene marvellous

What is challenge about diabetis in Malaysia. Mainly diet. This is due to Malaysia is multiracial country. Multiracial=multi cultural= variety tasty food. Most of variety food is not cholesterol friendly or diabetic friendly.
Most pt like NASI LEMAK, MILO, rice or king of fruit durian. This equal to bad sugar control. The challenging part of doctor is if you ask them to reduce those food intake is equal to IRRITATION . If you ask them to reduce rice intake, they will shout to you then what should i eat. If you said vegetable( 2 green), protein. They will said i don't have teeth to chew meet and vege. The best part is if they are warded , they will comment the meet is hard like stone. the vege is not fresh. The food is lousy as a result force to eat nasi lemak.

This is the normal accusation of pt and anti of diabetic diet. Really take time to counsel, estimate time 20 minute and above. First need to deal with their anger. Next....... Exact way of breaking the bad news. The bad news to them is their lost of their love ones-high sugar content food.

the answer is no. I am not the trekker. Trekker is a person addicted to star trek. I hardly watch any episode of star trek but i went and watch the star trek at first day primierre. Why?
The reason is that day the weather is too hot for me to sleep so the best place is went to cinema to get yourself tired. Secondly, the preview and reader review is good. Yes , i like scifi movie especially the preview of showing the planet is destroyed is cool. next the actor staring is the villain of Heroes-Sylar as good character. Third one, it is the beginning of USS enterprise.
I sort of start to like the beginning of each movie now and after star war, everyone started the beginning -movie, such as wolvelline-the beginning, texas chainsaw-the beginning.Even underworld 3 instalment - is the beginning.
Look like world is adapting the retrospective episode more and more . Good point, if you think the movie is good then watch the rest.By then, the boost of sale of dvd of old movie will increase.
We, as the dr, also always ask pt how the disease is started . Does it equal the beginning . Yes. If pt good at telling the beginning of symptom, diagnosis mostly should be correct. A lot of pt do not know how is started.

Finally, cool thing about star trek. It is humour, a bit scientific and special effect COOL. WATCH In theatre not your normal TV.

after hardwork and some failure that arouse me and bother me. I decided to watch the movie with my housework as titled above. A movie based on the book with the same name. It regards girl pick the wrong signal regarding man interested in her by wording from girl. It is really interested to watch as 1) starred by famous actor and actress 2) things we should be noticed when we talk to girl . 3) girl should watch as not picked the wrong signal.
what it applied to life? To me, careful with your words when you talked to someone. Other way of quoting , mind your language when you talk.
I think as a doctor, we deal with patient daily. Sometimes pieces of advice will be taking as scolding. Different tonne with same sentence and meaning may bring different meaning. It just like this movie said for example ' it is nice to meet you' bring different meaning. If it is stated at beginning it means the guy interested in you. If said at the ending , it means no, no, no.
I think when dealing with patient, maybe we should predict the response when we said something. So maintain eye contact to monitor response. Always apologize if you sense tense or anger in patient as patient always quite sensitive as they are sick . sick+occasionaly long waiting time = ill tempered most of the time.

2 days ago, i got a surprising call from my good friend in college. He is attending a cardiac conference and meet me. It has been a long time since he left for private then rejoin back government after membership. It took him about 10 month to join government as contract. This shows how 'fast' ministry act towards what so called 'shortage' in medical field.
Main thing we do is chatting. I like chatting face to face rather than talking via phone or internet. First method is expensive whereas second method is a bit slow due to our internet is not so advance. Furthermore i like to see friends.
Recently, friend of mine finally pregnant . Congrates pal.

madagascar impact

After watching madagascar ii, i am delighted to watch the story of Alex the lion, zebra, giraffe dr ,penguin ,hippopotamus and monkey. I am eager to watch the real lion, zebra and giraffe. As a result, i wake up early for Zoo negara.

right at the beginning, i saw giraffe busy eating the leaf, later i saW THE LION LYING lazily doing nothing. The lion hardly move compare to tiger busy walking around. zebra is with troops and so on with plenty of animal.

After that watching sealion show. the trip is pleasant but just a bit hot.

although already in this town for almost 5 month , i still lost in the town for new location. When ask people or friend for direction, i still tend to lose. Reason 1 - i am too dumb. Reason 2 , i can't figure the road or map in my brain. Reason 3- Kl people teach you drive without proper landmark. they like to point drive straight, turn left, right etc. At the end you miscount one turn, you end up lose in town.
my record or lose when my old roomate came from kelantan to meet me, i ended up lose 2-3 hr in a simple location.
With the advance of technology and net plus influence of my friend , i buy my Garmin on line and receive it today. I test it to the place i not very familar. The outcome is marvelous.
This will be my precious for a 'blind' man not to lose in town.

what is one the best sound track in this year? I think it is HSM iii . The songs is very refreshing and not too noisy. Second camper rock. Others sound track is not quite good this year. disney produce good sound track. The only sound track which i like other than Disney will be LOTR trilogy(lord of the ring). I may be biased because i like LOTR.
As usual , the best experience is watch that movie and listen the song in cinema. So, HSM iii , i spent my money to watch in cinema even though anticipate a simple story.

Finally, simple is good. Simple language easy to understand. Simple song easy to sing.Simple medication (like PCM) like by GP and patient(patient request it even you do not want to prescribe(they call it KK). But simple referral may be irrititaing(if too simple).

Monday blue vs excitement

What is monday blue? It is not medical term. It is not supporting chelsea(nickname the blues) at Monday. It means sadness or mild depression during Monday working. it happens because you are having too much fun on Saturday and Sunday. it happens to everyone no matter how you like the job. Then why Monday excitement.
To me , evrything got pro and cons. After waking up early for your job. After hard work, you found that all your series back on air on Monday. In Malaysia, all your hk series back on track.the fun of continuation. watch on line or using....... to watch the series. Excitement get better if you plan to watch prison break. it is back on air for season 4. What Scofield will do for next step to steal the card access to the company?
More excitement started on tuesday US time , House restarted the season 5. I can brush up my thinking towards medical diagnosis. A way of laziness yet learn some medical.
trust me on that , everything got pro and cons. Saturday and sunday movie is bad all the time because this is the time for you to have fun outside. Monday series is good because you are too tired to go out at Monday after work. Tv programme is rewarding.

I like to drink milo, tea or coffee at night. I do not have effect of insomnia by coffee or tea. 1 week ago, my green colour mug (Madopar sponsored)broke into half due to i accidentally pull a cloth from table . I am so depressed because this is my precious Madopar mug. Although it is free, but it remind me of the gold standard treatment of Parkinson disease. Today , i got one mug from another drug company. It is also green in colour .The name is Legalon.Sound a bit like legolas(lord of the ring). What it do? it is a liver supplement . Crowd in KL cinema

Today i will show you the biggest crowd i had ever seen in KL cinema. The location is mid valley megamal. You need to wait 30 minute for a movie. Why so many people today ? The reason is simple because there is about 4 box office show simutaneously. Batman-dark knight, hellboy,red cliff (directed by John Woo),and specially 3d movie-journey to centre of earth. I watched red cliff for 2nd time because my favourite ex roomate came from Kelantan KB (where there is no cinema) for coarse so he wants movie. I bring him to the most crowded cinema to watch movie and experience the dts surround system. Reason is clear so that next time he won't only watch pirated or original dvd. War scene, action scene, dark scene need cinema to get the effect. Watch dvd will spoit the fun and excitement of movie. i watch transformer in both cinema and crystal clear dvd and i found out that watching in dvd is less exciting. Today i prove it again by watching red cliff 2 times, one times in leisure mall , now in mid valley. It still interesting to watch because war scene still good to watch. So what the advertisement said is correct. Even wall E know pirated dvd quality is rubbish . i will said watching both pirated and original dvd at tv is rubbish especially for commercial movie and action movie.It is exceptional only when it cannot available in malaysia cinema or the movie is damn many talking like The hour.

1 week ago, i went home at friday. it proves to me that pudu is a a place full of crowd and kl is crowded because of outsiders.everyone rushing back to their home town to see their parents, lover , children etc. Bus ticket to penang can went up from 27 to 40. faster than petrol price . a lot of people already went there with luggage so they have no choice but to comply. JPJ action towards rising price is useless in that hour. picture above showing the crowd. picture below showing transnational departure time and platform in their own centre. Change to get transnational instantly is very slim. Advice to people please plan and buy your ticket few days before going back.

Today , went to berjaya times square to watch doomsday cause it is not available in leisure mall . I like 18PL movie and i know this movie won't be nice in Tv because preview shows the movie got 80% is dark scene. Dark scene in Tv equal to watching your own reflection or adjust too bright not realistic. Action a lot in this movie plus chopping of head. That's why 18PL but do not feel 18sx why said 18PL. never mind i am not finas. Why bad gsc counter service? Today i watched the 11.20am. As usual, counter opened late and very crowded to wait for counter opened. It only opened at 11.10am. Every one already line up for long then got one indian gsc counter attendant stand outside said buy ticket according to counter(each counter stick one A4 paper about one movie). A poor lady line up behind not knowing the system ended up at the wrong counter which is not the movie she want to watch. She beg that attendant because movie going to start but attendant ask him to line up in other row. I wonder why is this type of attitude appear on service business. You should try to serve and make people happy with your service. if not, just go and become police or thief..... . Anyway occasionally i think back to myself, there is so many demanding patients in our medical career nowadays but i just keep quiet and swallow my unsatisfaction. Why can't he do that.
let me share with you some little story. On one afternoon, that is a prof(islamic study) came to review my boss's clinic but my boss not there so staff nurse ask me to review the patient. Knowing that , he will show unsatisfaction but actually condition worse than that. He ask who am i to see him and ask me where is my boss? This is very insulting and embarassing . This is simple if you don't want other doctor tell early not to keep your mouth shut at the beginning then only talk nuts later. Another things is there are a few stupid staff nurse which is not aware of the so call somebody's personal patient. Please let the respective doctor saw them for god's sake before they start barking at people again.

For past 1 weeks ,i had personally broke my record of watching movie. In cinema for 3 times in 1 weeks (5 days precisely). 2 movie yesterday and today. Mood happiness ++ after work. During work pass 2 days, stress++. Referral case had been increased tremendously so go back almost 6 pm and above daily.So as a stress remover, past 2 days , i went for Hancock and Get smart. 2 days before, wanted. I like wanted action extremely, excluding Angelina Jolie sexy look and good acting, it is till a good movie. I will said is a action + thriller movie. Some sorts like matrix but a bit more realistic and less puzzle concept. Get smart is a good comedy for those who like spy comedy. Some big name in side the movie. Spy commedy to me =priceless esp after stress. Hancock is attract me the most is Will Smith. I can watch him fighting zombie alone in I,legend so why not he as a super heroes faced by many problem and hated by public.
next movie i am waiting will be the dark knight and red cliff.

First movie in new town.

after 13day in new town ,KL . I finally went to watch my first movie in new town.
KL give me the impression of 4 lots-lots of traffic, lots of person(congested),lots of money(to survive),lots of road(NO u turn).
Still do few u turn before going back from theatre to my house coz not familar to the road.

What movie i watch. Sure my favourite marvel comic character'hulk'. My female colleague hate this chararcter coz he is ugly and big. I like it coz he is the tragedy hero.Unlike spiderman, he is more tragic than spiderman.
Spiderman need to cope with daily life.Great power comes great responsiblitity for spiderman. Hulk need to sacrifice his love, need to meditate and foremost need to control his anger.

I think everyone got the hulk in ourself. When we get angry, we actually already change to HUlk not morphologically (physically) but emotionally. When we lose temper, occasionally, we said something bad that may make us regret for whole life. We may make people hate us or worst still lose our friend all together. Imagine when a patient start provoke you or irritate you, occasional you feel angry and sometimes lose control.You answer back. That's it ,after that you will face complaint and unnessasary condition. So we must learn to control the anger .Just like Banner learn to control hulk in him.

After doing my master in Penang for 2 years, finally back to campus HUKM in Cheras. KL is a strange place to me. Road is very confusing . The sign board point to you one destination. After driving, the leading sign lost in sign board. It is confusing.
people driving here a bit funny also. They try to dive very fast during jam such as overtaking one car which can be fast for 3-5 sec but drive extremely slow during no jam time.
I still get to know their attitude.Luckily i only need to walk to campus.just rent a house behind.

Although the soul of Indiana jones (harrison Ford) getting older but i still like it. It is indiana Jones that make all the treasure hunt movie coming one after another. It followed by national treasure, tomb raider and etc. Indiana john 4th series still continue with the treasure story with the background of russian as villian . They try to resolve with crystal skull. The story make jokes of Indiana jones being aging and not as fast as a young guy any more occasionally. The story still good. I still like the temple of doom and last crusade. Maybe the action is more. I like this movie because it is sincere. They portrait what will happen if indiana jones get older and how he cope with it. So i still like it even action for indiana is less. The other supporting actor is equally good.

What is the cause of short circuit. Normally people saying about water,exposing wire. How about ant? Believe it or not?
I got one jug kettle for boiling water. I go back weekly to my home town. The next week i came back i found my kettle(the basement) fill with ant. When i opened the electricity, the power plug is off.(tripp).. i called the technician, the technician the kettle got problem. Out of curiosity, i opened the cover of the heating basement,to my surprise, it filled with thousands of ant(dying and alive). I slowly picked up the corpse of ant and clean up of the ant. At last, my jug kettle refunction again.
Cause of short circuit is ants.

everyone talk about vista. I could not resist the temptation and end up using vista because the colour is much better. What is the side effect. Computer get very slow.Need to use the office 07. What is worse about office 07 is even the saving feature is hidden . No more save button outside. This is same as explorer. I still prefer firefox and my old office application. another irritating thing is every installation the window will ask for permission such as registry going to be changed . Are you agreed or not. To the dummies, it is confusing. To me ,it is irritating.
Maybe after certain period i will like it. If over the period of trial, i still not like it then i will need to reformat my computer and back to square one(my XP).

I went and watched street kings yesterday. It is because i like the preview. keanu reeves become the antiheroes. In the preview he do the false evidence after killing people. After stay in Ipoh for almost a month. At last, i go and buy Ipoh famous snack. They are "kai Chai Peng", "Lou Poh Peng"(wife biscuit) and Ipoh Charcoal cooked "hiong Peng". I like Kai Chai Peng the most because it is yummy and crispy.Lou Poh Peng is not bad either .Ipoh Hiong Peng is not so special . I still prefer Penang Hiong Peng.
Loh Poh Peng in Ipoh is different from Hong Kong one in the sense that Hong Kong one more yummy. Hong Kong Loh Poh Peng got tong kua tiau inside whereas Ipoh one got sesame. Any way it is good to have something to chew during pastime and can increase my weight further. He he.

For past 2 week, i had been watching movie in each week. One movie per week. Last week , i had watch an empress and the warrior. I like the song sang by leon Lai and kelly chan.here is the music video.
Do not mistaken that i like Leon Lai but i particularly like Kelly chan 's song.The movie is about a king who is murdered by the nephew who wanted to become a king.The king wanted Donnie Yen (the top warrior ) to be his successor but the nephew not aqreed with him and try to make riot of that. Kelly chan forced to be the empress. She get injured by the assassin sent by the nephew.She is saved by Leon Lai and fall in love with him. At last.... The movie fighting scene is good with the message war won't be ended unless somebody let go of it.

27 dresses is the romance movie staring by Katheline Hiegl (one of the grey anatomy Dr) .The movie where she is the bridemaid for 27 times and find the true love at last.The romance movie with some new elements in it. It is the so called medium class movie . Selling point is the commedy and romance. Recommended for person who is under a bit of stress yet can't release it by other means.Plus no good movie to watch at this moments.Persaonally looking forward to the forbidden kingdom by Jet Li and JAckie chan.

It had been second week in Ipoh. Things and food here not seems to be cheaper except movie. This week met with another chan TM(our colleague) who join politic and maybe our future Mp. He is doing well in his clinic and soon to be father of second child. AQnd another of our senior (my past heroes enterprise elite member) NKC . He is also Gp in Ipoh. They are both doing well.
Back to movie. frankly movie here is cheap so i ended up watching movie here. RM 9 and RM 6 for special day. I had watched Ah long and spiderwick . For details please click to my movie link,
I recommend ah long ltd as it is good as above.A typical Jack Neo movie who making jokes out of Ah Long (loan shark). One theory inside is Ah long also is the victim. Why? Because they borrow money but cannot make people into Bankrupt as bank and take away people's house as what bank had done.
Spiderwick is the fiction i always enjoyed.

I am now doing dermatology in Ipoh Hospital. It has been a week being there. Having a bit of adjustment disorder mainly because been long working at same place for past 4-5 year. Luckily the colleague here and the boss is very friendly. There is my old course mate in U there,Chan 5354. he is now doing paeds still as handsome as previously.
First day stay, feel a bit alien to this place. Rain at evening thinking is a coincidence. My 6 days stay prove that it is a raining day almost for past 6 days. Is this just like Taiping? A city of rain? I like rainy day because it make me sleep well. Cons is i slept early everyday and still feel sleepy.
Another thing is i feel a bit depressed coz i do not have internet access. I missed my streamyx.
I came back to my sweet home today at last after the derm coarse to watch football (EPL) on TV and play the internet i missed so much for past 6 days.
Food wise i still feel Pg food is the best although people claims Ipoh is the paradise of food.
Road wise i still occasional lose during driving . Luckily chan help me a lot in recognize the route here.

After nomination at 24 th Feb 2008, it marked the beginning of preparation of election. Every candidate try to get votes from their boss (us the voters)for this thirteen days before they rule our future. Just like people in love, now they promise us everything. Every party using their own tactics to get the votes. We are the one who cast our vote and determine the future of nation. Some of the voters votes according to party, some according to beauty (a lot of beautiful candidate), some based on capability of candidate. some follow candidate. some follow the crowd. Some only decide at last minutes. Since election is round the corner. I would like to share with you a jokes as below.

While walking down the street one day a politician is tragically hit by a truck and dies. His soul arrives in heaven and is met by St. Peter at the entrance.

"Welcome to heaven," says St. Peter. "Before you settle in, it seems there is a problem. We seldom see a high official around these parts, you see, so we're not sure what to do with you." "No problem, just let me in," says the man.

"Well, I'd like to, but I have orders from higher up. What we'll do is have you spend one day in hell and one in heaven. Then you can choose where to spend eternity." "Really, I have made up my mind. I want to be in heaven," says the Yang Berhormat

"I'm sorry, but we have our rules," says St. Peter. And with that, St. Peter escorts him to the elevator and he goes down, down, down to hell. The doors open and he finds himself in the middle of a green golf course. In the distance is a clubhouse and standing in front of it are all his friends and other politicians who had worked with him.

Everyone is very happy and dressed in the finest batik there is. They run to greet him, shake his hand, and reminisce about the good times they had while getting rich at the expense of the people. They play a friendly game of golf and then indulge themselves on lobsters, caviar and the most expensive food there is.

Also present is the devil, who really is a very friendly guy who has a good time dancing and telling jokes. They are having such a good time that before he realizes it, it is time to go. Everyone gives him a hearty farewell and waves while the elevator rises.

The elevator goes up, up, up and the door reopens on heaven where St. Peter is waiting for him. "Now it' s time to visit heaven." So, 24 hours pass with the Yang Berhormat joining a group of contented souls moving from cloud to cloud, playing the harp and singing. They have a good time and, before he realizes it, the 24 hours have gone by and St.Peter returns.

"Well, then, you've spent a day in hell and another in heaven. Now choose your eternity." The Yang Berhormat reflects for a minute, then he answers: "Well, I would never have said it before, I mean heaven has been delightful, but I think I am better off in hell."

So St. Peter escorts him to the elevator and he goes down, down, down to hell. Now the doors of the elevator open and he's in the middle of a barren land covered with waste and garbage. He sees all his friends, dressed in rags, picking up the trash and putting it in black bags as more trash falls from above. The devil comes over to him and puts his arm around his shoulder.

"I don't understand," stammers the Yang Berhormat. "Yesterday I was here and there was a golf course and clubhouse, and we ate lobster and caviar, drank champagne, and danced and had a great time. Now there's just a wasteland full of garbage and my friends look miserable. What happened?"

The devil looks at him, smiles and says, "Yesterday we were campaigning." "Today you voted."

At last i watch my targeted movie this year 'the jumper'. Jumper as the name is the movie about a person can jump from one place to another place. Example can jump from US to library or to other place in the second. This is a awesome movie cause it is a total new idea of movie. What you want to do if you are the jumper. If you can jump from one site to another will you steal money from the bank. I won't but i will travel round the world and i will be very happy because no need waste time on traffic jam and can on call from house and just jump the emergency scene when people page me. Movie on 1st day new year, i planned to watch CJ 7 but due to huge crowd. I ended up watching kung fu dunk. Not a bad movie, kung fu dunk is a movie by Jay Chou. A movie about a orphan who got kung fu ended up playing basketball. A movie combining kung fu with basketball. A bit different from shao lin soccer but still a good movie to watch. Entertaining and relaxing to watch in new year.

Tomorrow is our chinese big reunion dinner. Those who stay in oversea and outstation, i guess already packing or already went home. I am just working in local so do not feel the excitement to pack and go home.I ended up become victim of bully. Not getting the leave tomorrow. Anyway, this year is a happy year. With the help of facebook, i found the sexy big mouth Jason(my old course mate),the future minister (ong Ka teng alike)Ho, my junior Hui, my heroes buddy Liew and many more.
Anyway, today i sent all my chinese patient back for reunion and hope that those who ill with get a bit better and go back tomorrow. It is because abalone,siewchu(bbq pork), chicken, sea cucumber waiting for them. Plus, their son, daughter also waiting for them. To those homeless or abandoned by their offspring, too bad for them. They may be ended up in the ward for new year. Having reunion dinner with other patient. This issue have been mentioned in my previos blog last year.

in my hospital, what i like the most is going to library at meal time because it is the time when after you take a meal and nothing to do. Too far to go home . Too lazy to see patient in ward because patient got visitor to see. Unless emergency in ward , if not , i won't disturb their time with thier love one. I would just hang around and surf the net. Read some article and check for football results particularly those games play on midnight. see whether worthwhile to see the delayed telecast or not.

computer defective yet expensive?

What is this topic about? Actually , a friend of mine inform me of worm infestation for his computer and make his computer cannot log in. Yes, worm infestation for computer no more computer virus. Computer worm more bulky, more damage done, more difficulty to fix.
So for computer user like me always pray not to get worm for computer. If i got one in my computer , i will format my computer (i previously got one). After the worm, I kind of got Obsessive compulsive disorder and backup my data at weekly basis.any worm attack means one weeks work gone. Always two backup(one in portable hard disk, another either in thumb drive or dvd).
in conclusion, computer is an expensive item but defective because it is vulnerable to worm,virus attack. just like us , easy to get disease if we are not keeping well.

A good movie i had long watched but not write review. It is about Spartan attacked by massive army of persian. At last all of them dead. In exchange of their dead, the council approve for the army to fight Persian.
What is the reason of Spartan to fight massive Persian with small numbers. The reason is bribery of witch, the hatred of one of the council member to Leonidas(the king) and the love of Leonidas 's wife.
Overall is the movie should watched. Even the story is slow and not all about fighting only.

even i know that resident evil is the junk movie. i still went and watched it at cineplex because this movie need big screen to get the feel of the movie. This movie exactly like the computer games of similiar name. At first movie, Alice still not perfect in her fighting skills but in third movie. She only need to use two blade,or even bare hand to kill the zombie. She can kill the swarm of mutant crow with her super
skills by staring. Believe or not, she is just become more and more god like.
It is logic as when you are playing games, you got upgrade and super skills. As a movie, it is a bit ridiculous.
That's why i called it junk movie. It is just like junk food.( No benefit of eating them but fun of eating it.)similiar to this movie, after you finished watching it , that is not much to say about the story but excited during watching it.
Another reason i watch it is because Milla Jovovich and Ali Larter act in this movie.
I support Milla since she act in fifth elements. Ali Larter because i supports Heroes. (similiar name like the game i used to play).
Actually i think the first resident evil is the best. This one is really junk but fun to watch.

is heroes another x man? In the series , actually there is a character got the similiar ability like Mystique (a sub character work under lindermann), Molly walker, a girl who can track all the people with special ability . She is just like prof X in xmen but she don't need to work under the big machine to track people.for me , she is the GPS system .Sylar who acquire people power by opening people brain , a bit like hannibal 's style of killing.He merge everyone power and to become the strongest . He is a super villain in the Heroes. he got the power of telekinesis just a bit like Jean in x men.
Nikki acquire the super strength just like wolverine. The regeneration of wolverine also found in Claire.
Nathan got the power of flying. He is another superman or storm but just do not have super strength and cannot control the weather
Actually heroes is another story modified from X men . The difference is they do not wear costume. They are not a super heroes. They are just a ordinary human like you and me. They got their personal problem also. Most of them do not like people knowing their power. They do not want to become freak.
This makes heroes interesting and fascinating.
It makes us wonder if one day i got the power what will i do? If i can bent the space and time , will i use it to save life. Will i become the surgeon ? Just in case some one bleed during op , I just need to freeze and then suture it back. isn't it cool. I can freeze the time to study before exam so that i got more time to do other thing.I can teleport to working place.

I wonder what will i do if i acquire claire's power of regeneration, would i be like her ? Would i try to save everyone in danger? If i got the power of Sylar, would i be like him or i would rather sit back and repair the watch. If i can bent the time like Hiro ,would i try to save the world? that is so much if in this issue? I definitely won't be like Sylar or like Hiro. I won't expose my power just in case i would be killed. I will use my power for good deed.
Anyway , heroes season 2 already started. just sit back and enjoy!

after doing round in ward for 2 consecutive week with calls. Basically working 7 days per weeks. it is very taxing and kill your interest on studying and working. You are too tired to read the review. Basically got some depression mood.
Luckily, my weekend at last is back.
A weekend for me to do movie marathon. I watched secrets by Jay Chow in cinema, surf up and simply actors in dvd. it is just happy and relaxing.
secrets is a fantasy romance movie with the back ground in school. Simply actor is the typical Hong Kong commedy movie . Surf up is the fair animation. This combination is one of the best relaxation therapy.
I am so happy because i manage to finished third season of desperate housewives and now working to finished the first season of heroes.
In conclusion, every one need weekend to relax either watching movie, shopping, or taking a good meal. The relaxation is based on your own interest. After the recharging, then start the new days with good mood to work in Monday

Our HO room got thief/thieves?

recently when i am doing round . My Ho told me that his colleague in cardio ward loss the precious hand phone in the Ho room 2 month ago. just a week or 2 week ago in the same room, another houseman loss the wallet again with money. This is the second time in the past 3 month. They kept informing me knowing that i like to keep the bag in the room .Maybe worry i got post traumatic stress syndrome if i loss my money and ended up scolding them?!!I keep my bag in the ward because it is near to my ward(nephro ward).Actually , who is the prime suspect?
As i know , this room only visited by doctor, Ho or MO, occasional medical students, attendent who is arranging the bed ,support service staff and diet staff.Who is the thieves? I do not know? I found out my bag always misplaced when i plan to go back. From table to chair or to floor. I think my bag had been treasure hunt but found nothing precious.
I actually do keep my black box there (pager) . Guess it is a cursing item (cause low morale in heroes games) so it never got stolen.Occasionally got stethoscope, guess too old so it is not stolen.Some medical journal , guess too dull to be stolen.

Who is the thieves ? I still do not know. Actually if i put my handphone maybe, it also won't be stolen because it is only Nokia 3100. it cost less than 150 in secong hand shop.

the thief issue is always arise in ho/mo room, or in Ot in almost every hospital.
When i become ho in taiping Gh,My colleague (dr 5354) who i think fibrate knew him loss his wallet at O& G OT when assisting the Mo for emergency caeseraen section. It is because the case is too emergency and the MO too fierce, he is too hurry and ended up forget to take his wallet with him and let the wallets remains in the pants.After finished ot , wallet is gone.

I guess every hospital got a staff or more constantly checking our bag and dress of doctor every now and then. They (thieves) know that saving life is a priority and property is only second in priority. Actually i got loss my watch during doing procedure last year. i take off my watch for hand washing then i forget to keep in pocket , it loss in 15 minute time.Luckily it is not the expensive watch like Rolex, Tag hauer, Rado. It is only swiss hunter watch cost 200 over. But it is still traumatising to loss your property.

Compare to my friend who take dinner with me 4 month ago . I consider my self unlucky. He left his handphone at coffee shop table after dinner. After 15 minutes time of ride , he told me he left the handphone on coffee shop table. Do you think he can found back the handphone? Logically no isn't it.
But he got his handphone back in the same table. Nobody seems to interested in his handphone. Guess what,his handphone is nokia 3310.

From there, the conclusion is the thief only pick on item which is popular and can be sold. stupid item won't be stolen. Spiderman 's uncle told spiderman great power come the great responsibility .I would said great item come the great responsibility.

So, beware for those using smart phone or great phone, make sure they are following you even in toilet unless you lock it in locker(maybe got spare key also). Or else , you will be low morale.

Perfect stranger is a recently watched movie from the borrowing. It is a nice movie . Wonder how i missed it . Maybe too busy at that time. A perfect murder is the right incredient ,right person at the right time. Quoted by the main actress Ro(played by Halle belle).
For me , perfect stranger is the good movie combined with perfect actor (Bruce willis) and actress (helle Belle) plus good story line and twisted of story.
You won't know the ending until you watched it till finished, i called it brilliant.

pass motion does matters!

I have been in medical for quite a long time . One of the most irritating complaint and frightening complaint is about pass motion. Every morning, when i ask my patient are they feel ok.Some of the patient (especially old one ) will tell me they are Ok except they cannot pass motion. Maybe already for one week.So i will need to squeeze my brain to figure out is it the severe intestinal obstruction, or is the stool harder to pass out,or passing less than normal. Do not think i am mad !I got one patient complaint of not passing motion . On further auestioning, the passing motion in a day is less than three.(only two times) . He normally pass two times.Some pass hard stool also give a complaint of not pass motion.
basically unable to pass motion is a tiring business .When you try to get further confirmation they are not happy. They will tell you to just give them laxative. malaysia patient hates diet advice for hard stool. When you ask them to take high fibre diet to get the normal motion after you rule out the potential life threatening causes of unable to pass motion(such as intestinal obstruction,). They will be super unhappy. They will make the statement as below:
Doctor: I have examine you everything is normal.
Pt: why can't i pass motion as usual.
doctor:Maybe you need to walk more and take high fibre food to get the normal motion back .i see you sleeping all the time.
Pt: I have nothing to do so i am sleeping.What is high fibre food?
Doctor:More vegetable and fruit.
Pt: the vegetable in hospital diet sucks. Just give me laxative.
Doctor: Laxative may make you pass too much and ended up diarrhoea.
Pt: Just give me the laxative!
Normally the conversation will ended up with doctor ended up giving laxative after one or two days because it is getting more and more irritated.
After laxative given, i considered my nightmare will begin soon. This time , they will complaint like below:
Pt: Dr, why my passing motion so many times one. today already ten times already.
Dr: you ask for laxative and i already tell you the side effects.
Pt: still it is too many times that i wanted.Dr, you see now i am dehydrated already. Please give me medication to stop it.
Dr:if i give you the medication to stop it . You will be constipated again. I give you ORS (oral rehydration salt) tp hydrate you back.
Pt:(appeared angry) Look i am going to die already by diarrhoea. I hardly got time to sleep . Now you ask me to drink ORS in fact you can stop the motion. Are you kidding?You have to do something!

See, this matters is going on and on. It will trouble you everyday.
A patient come with diarrhoea also will crack you head for diagnosis. There is 101 cause for diarrhoea. Especially those with polypharmacy . A lot of medication can cause diarrhoea. Need to rule out pseudomembranous collitis, thyroidtoxicosis , HIV and etc.

In conclusion , pass motion business does matters. it can be small or can be urgent. The outcome is big headache for the treating doctor.

Pc fair is back to my town recently. as usual,it is the most wanted place for tech savvy like me to go. It is normally populated by male population because the products normally are traded in cash and carry. As usual , i normally will visit the place to buy some products without rally needed it. i ended up buying a extra hard disk portable one and dvd blank disk and pen drive without really needed it. What we called wasted. What i called moving and stimulating economy in tech business.
the most tempting thing in this fair this time is pocket pc + phone. Which i not really need it but really plan to get one. The price is .....super high. HCT touch 1999 and the latest toy pda phone+ tv is 2099(giga i smart). Really dream to get it. Due to previous painful experience of losing one in 2 years(cause of pda death-unknown.)I resist the temptation and will wait for the price to fall until i happy with the price and feature. What my friend comment me as stupid because losing the maximum satisfaction when you have one now.
Anyway, the place is such a jam just like a traffic jam but the difference is only by people.Every one bargain and buy the item happily. I think a lot of them just buy for the sake of others buying .
Any way i still happy to go there because i saw two of my old class mate there. It is not a bad place to meet people there especially old friend .

Out of sudden , my palm T5 malfunction start from friday. It could not restart either by soft or hard reset and basically it is frozen at the loading screen. I broke my bad news to my colleague. My colleague tell me it's time to change. Actually it is not money that matters the most. It is just the sentimental value , My palm has accompany me for past 2 year. it is not the short period and offer me a lot of info i needed during emergency and quick drug reference.
Basically , if i got a new pda, i need to restart everything again (reentry data) or maybe i should keep a simple organiser(diary or note book).

currently that's a logging problem for my computer to blogger so i started to watch a korean series "devil king". or so called 'mowang' in mandarin. It is a series between a lawyer and police. Actually the police got the background of killing the lawyer's brother during teenages. Due to the political influence , the police is not guilty in trial. The lawyer's mother died of accidents on that day. Out of the hatred , the lawyer start revenge by indirectly control third party to kill the police's friend one by one.The lawyer switch his identity with his friend who already dead. the story is very interesting and is 20 episode.
Highly recommended to watch because it is full of twist.
this is why i not writing blog for this few month.

harry potter 5 is fair.

As a 5th movie of harry potter, it is not so excited compare to previous 4 except the last part. The centre part basically about umbridge try to catch the secret organisation 'army of Dumbledore' lead by Harry. only at the prophecy part, it become excited.
Only one part we can learn is we must defend the truth and trusted the truth. We should let the bad and powerful people overturn the truth.Something pretty valuable we can learn from harry, dumbledore. We need friend to get stonger.

die hard 4.0 with lessons!

I watch die hard 4.0 yeasterday. It is awesome. Selling point is 'firesale'(virtual terrorism) and hacker. Just like transformer. Hacker is the main focus of the movie.
In the movie, virtual terrorism so called firesale (everything must go) .The attack focused on four primary objectives: disrupting transportation, stealing and destroying financial records, disabling all public utilities, and creating fear with a PSYOP media campaign. The attack is more traggic than 911. It not only involve life but involving bankruptcy of the government. After looking at the movie, it seems to me that using computer to control everything is not a good idea after all. I think human should be involve in controling and can be manually override everything if computer being hacked and situation in chaos. The action part is still good and packed. Selling point is a chaos created by firesale. The traffic, subway, biological attack, finance and psyop. I like the movies because that is progressive improvement in terrorism for the movie . Die hard 1 is fighting terrorism at building, die hard 2 is fighting terrorism in plane, die hard 3 is robbing the high-security vault in the basement of the Federal Reserve Bank of New York with the diversion of bombing New york. Die hard 4 is stealing money by create fire sales.
If that is 5 maybe fighting a biological warfare or cyborg?
My advice is watch the movies .Lesson is do not let computer control your life.
Actual facts is that is no John McClane in the world.

it is hard to believe that this year is a movie year. All the blockbuster is available this year. Start from spiderman 3,then pirates of carribean 3, then shrek 3 then ocean 13.Started from July remaking of transformers from animation.

bourne ultimatum(finale of bourne identity and bourne supremacy),
I am a legend by will smith (another action movie by will smith),
the entertaining and funny rush hour 3,
underdog( a dog who got super power),resident evil 3, the invasion by Nicole kidman.
That's the top ten movie for the second half of year started from transformers.
Looks like i need to spend more money for movie this year.

Driving issue

Recently on my way home, stuck in traffic jam . i always listen to 988 (chinese station) . It is not because i like the dj . The main reason is the driving etiquette and issue is aired.Today issue is about what you will do when you accidentally scratch people's car and the people is not around. I will leave my phone no at that site and record the car plate no but i normally become the victim rather than become the knockers. Previously , the issue is about back seat safe belt. A lot of people sms they not used to it. A lot safety issue discussed. Sometimes they tell you how to take care of your car. Ideally ,you need to start your engine for 3 minutes at early morning before you start to drive to protect the engine. I never do it.Overtake should done from right side. This all seems simple but i find it useful to listen particularly during driving. This is like growing good habits during driving.

After i watch transformers-the movie. I'm typically fond with the music particularly the ending of theme by Linkin Park. That's why i stayed back and watched the credits and saw the cutscene. I liked the what i 've done performed by linkin park(ending theme).Before its too late by goo goo dolls is another good song played in the movie.Other song like disturbed-this moment and others is also a fast and good songs.
Another good song recently is beautiful liar by beyonce and Shakira, Over it by ex american idol runner up last year (katherine Mcphee). Big girls don't cry by fergie. I normally listening to song during driving so the common song played with attract me to search for the song and listening to it during writing my blog.
Anyway i also like kelly clarkson, bo rice, carrie underwood . Their song is nice and sweet.

Transformer

Transformers is a movie i long awaited for this year. I am fascinated by the preview. I am one of the die hard fans of transformer during 80s . I got certain collection of transformer. Actually transformers got us and japanese version. Japanese version is more realistic and more 3 D in animation. The weakness is only the language. I actually watch the us version one. I went to see the movie today and actually quite impressed with the movie. Good storyline. Action one after another interrupt with some jokes. Friendship between autobot. The friendship build in between autobot and sam. Sam and his new girlfriend.
At last the big fight between Optimus prime and megatron. the cube(allsparks) that endanger the mankind. I watched the movie with a lot of adults rather than the kids. Maybe transformers fans all like to know what they do to the characters. I hate the director for changing the bumble bee into Chevrolet camaro which should be Volkswagen beetle. I think Micheal Bay hates germans(Volkswagen made in germany). At the second hand car shop the car beside camaro (bumble bee) is yellow volkswagen and is destroyed the windows and bodies. I think director hate volkswagen. Megatron also not transformed into the gun as original anamation. The music in the movie is cool although no original score of the transformers.
In conclusion , i like the movie but if they preserve the original tranformed form . I will be more happy.
By the way, for those who watch the movie, Please do not leave the theatre before finishing the credits. That is 3 cutscene in between credit. ONe of them show you starscream actually still alive.

I like it because it is simple

Simplicity is good. After a tiring week for the past five days. I felt such a relieve as it is saturday. At last, i got time to write my blog. Last sunday , i watched the Fantastic four-rise of silver surfer(fantastic four-2), it is simple and yet entertaining.
I like the fantastic four because all the four heroes are simple character. Invisible girl want to married and get a simple normal life. She do not want to become heroine.
Her brother , human torch want to gain money by being a hero, enjoy being a hero , like publicity and enjoy life.Mr fantastic still do his scientific work and research , bear a great responsibity for him self . Love invisible girl and trying to save the world . He not enjoy publicity.The thing loves the blind black lady and help mr fantastic. He like to protect him. They are all simple person. They are not like Superman bear all responsibility in his shoulder. they are not like batman although rich not still sad because he felt sorry for his parents dead and try to seek justice. They are not like spiderman always think great power comes great responsibility and over worried about MJ.
Maybe because they are four simple person who work hand in hand to fight for justice when they are needed. Although human torch always quarrel with the thing but they are still close to each others.
i like them because i think a lot of problem can be solved by simplest way. watch this movie and the fun is there.

I missed you, Father

After my father dead 4 years ago, it's always a pain in my heart on Father's Day every year. Father is one of my respective figure in this world. My father rewards me for good things i did and tell me when i went wrong. He never beat me for the damage i had done during childhood. When i am small, i like to open the thing i am not understand and will eventually damage it. I opened 2 to 3 radio before because i don't know why that thing produce sound without people inside. I always fantasied that is a small little man inside.
Father is a good swimmer who i learn the swimming skill from him. My father always played ping pong with me. Father encourage me to read books and always give me interesting story book to read. Father bring me to a lot of interesting place. He also teach me thing i not understand.Father tell me a lot of jokes and real story.
Dad, although you have gone from this world.You will always live in my heart . You won't be lose from my memory because you are irreplacible.

It sucks!

This week is a busy week and bad week from me. Continuous with the previous remaining patient from various country, that is still a remaining foreign patient in this ward. Other than taking over indon patient with Infective endocarditis to cardio ward and discharge of vietnam and bangaladesh dengue patient . The biggest headache patient is still in our ward. the bangaladesh patient which we take 2 to 4 days to get his stool sample. The best part is he got Salmonella typhi. So it is Typhoid fever. We started him with rocephine and to be discharged we need him to hand in 3 stool C & S(culture and sensitivity) which is negative. Basically he not understand what we want. so everyday we do sign language to him .Every patient happy to see that because we looks like "monkey" when we try to communicate with him . At last, that is a middle aged Bangaladesh who works for 12 years become our translator.He still refused to sent stool samples until further discuss plus forcing only he agreed. When i am happy and foresee the discharged the patient.There comes another Bangaladesh patient (not understand BM and english)with diarrhoea . The stool and blood C& S turned out to be Salmonella typhi . Oh ,goodness. Another monkey moments for us.
I think government should consider teach us the simple bangaladesh .At least , set the rule for all bangaladesh worker to know the simple BM or english before step in Malaysia . It is troublesome to us since Their commission is not helpful to provide translator to us. Not many people understand bangaladesh worker.Another thing is they are really a major host for Typhoid.
Almost all patient with fever, diarrhoea and abdominal pain is typhoid.
No other cause is the provisional diagnosis.
I wish i will got the ability of C3PO (the golden robot ) who peaks 290 language in the galaxy to communicate with them or archieve the ability of mind reader to know what they are thinking and sent my brain message to them.
Obviously , this is the fantasy.

Medical myth vs medical facts.

After reading my buddies post, actually other than the unclear concept of disease among the patient. Actually that is a lot of myth among the patient also.
For example , for fits patient. A lot of patient's family believe that when their kids got fits especially during generalised tonic-clonic fits, they should be giving something to bite. It can be chopstick for chinese, spoon for Malay or modern chinese (do not have chopstick),plastic comb for those can neither find spoon nor chopstick, Parker pen for one of my patient previously . A more educated one will use handkerchief wrapped the comb, spoon ,chopstick or what so ever for patient to bite. Pity patient occasionally break their tooth or got more serious injury in oral cavity. Luckily still not see people put in their little finger inside and got swallowed.
When i try to tell them not repeat it again.
The answer is: Dr, you know or not . If i don't do this. He is dead by now.
Dr: Why?
Patient's friend:He will bite his tongue and it will cause dead. When the tongue breed , sure dead one.
dr: which doctor or which book tell you this facts?
Patient's friend: Everyone also know this . You don't know meh? This is common sense.
See what they so called common sense actually is what i called a medical myth. Something no medical based but everyone believe. It takes a lot of time to educate them about coma position and not to put thing in the mouth. Sad to say that only the epilepsy patient 's family with frequent fits attack for the patient will convince after the recurrence enforcement education to them.
This is the first medical myth i encountered.
Second myth normally in TB clinic.
Dr: Pakcik, look at the X-ray ,see white spot at apical region like cotton wool(soft spoT) . Sputum AFB positive, You got TB (tuberculosis) and need treatment. Don't worry, it can be cured.
Pakcik: Dr, this is not TB . this is santau(black magic). That's why i got hemoptysis.Taking the medication will kill me. I need to see Bomoh.
Dr: If you don't seek treatment, you may be dead and endanger your family.They may got TB from me
Pakcik (a bit agitated):you are not malay. You don't know santau. Don't argue with me . I need Bomoh now . If cannot cure than may be TB.
Dr: I ask my chest physician explained to you. It is expect and is malay too.
Specialist told the same story.
Pakcik very angry: you all trying to kill me by asking to take medication. You know or not, I delayed to find Bomoh, i maybe dead.
this is the biggest challenge in chest clinic. To convince them they actually got TB. No people will believe you if they think they got black magic(charmed) even you are chest physician.
This again is medical myth in Malaysia.
Funny thing is they may involve a highly educated people (Uni graduated) among them. I just attempted one uni graduated high rank officer in 2 month back and refused treatment even signed the letter refused for treatment.
Medical myth is very strong in this part of the world. When medical myth contradict the medical management, they will opt for medical myth. I dicussed it previously in my previous post.
